IP Office 9.0.6 Cannot Dial Internal Extensions 2

Customer has been running IPOv2 with 9.0.2 for about the past year with no issues. They were initially installed at 8.1 over 2 years ago.
They started having issues where they could not call extension to extension as of Tuesday. External calling still worked. Only a reboot would fix it. They have approx 130 IP phones. I see no licensing issues or alarms in system status. We patched them last night to 9.0.6 and they are still having the issue. I had them try analog to analog extension to rule out network issues and analog to analog doesn't work either. They cannot get past the 1st digit of a 4 digit extension and it comes back as incompatible.

Anyone ever seen this before?
 
Are there any system shortcodes or SCN dialing that could be conflicting with the extensions?
 
No system shortcodes or SCN. Avaya recommended to restore an old config and see what happens.
 
Take it down to R9.0.4, 9.0.6 is not a good build.

We had abnormal termination reboots with about 7 customers on 9.0.4.
 
Pick one you prefer then (still termination is better than no calls at all:)

Check System>Telephony>Dial Delay Count - value must be phonenumber length +1
Check System>Telephony>Dial Delay Count - default value = 1
Check Extensions - is there a extension with only one digit starting with you internal number plan?
Check Shortcodes - is there a shortcode like ?D or a shortcode with a single digit equal to the first internal numbers digit?
Check the above two in user rights
Run Monitor and in the Call filter enable Shortcode messages, dial a internal number and post the results here
Asking to load a old config is dumb > not really a way of fault finding in my opinion

Sorry all. For whatever reason I stopped receiving notifications via email of the replies after Gunnaro's reply. I had another post from yesterday with a reply and no notification....I'll look into that.

Anyway, not sure at this point. On Thursday night, we restored an old config and rebooted. Out of 125 IP phones, only about 23 came back online. I was seeing System Unlicensed in Monitor when looking at H323 extensions. But 23 phone were up. Customer had to go to site at 10:30pm and log all phones in. This apparently happened when we rebooted earlier that day as well to clear internal dialing issues. Reboots before never produced this behavior. After all phones were logged back in, the system worked fine on Friday and Saturday. They came in Monday and couldn't dial internally again. This happened 3 times Monday and the customer was getting frustrated. We went onsite on Monday night and replaced the v2 chassis and also reformatted/recreated the SD card and then loaded a config. Everything came back up and phones all were logged in correctly.

No reported issues yesterday or today as of this point. I'll keep everyone posted. Sorry for the delayed response. And thanks to those that reached out.
