Intuity Audix R1 Transfer issue

When people call our main auto attendant they randomly receive the following message when they're trying to transfer to an extension:

a191 f158: Transfer cannot be completed.
f236: Try again
f109: later.
f95: Please disconnect.

Any suggestions to what is causing this to happen and how to fix it.

The Intuity Audix is R4 using mode-code integration with a prologix R6.
 
Just a thought.
Is the extension they are trying to hit a single line that is busy with no coverage path? Or is the extension not assigned?

The problem occurs when transferring to single or multiline sets with coverage paths.

Example.
Enter the auto attend with direct dial extension prompt.

Dial-in the extension 2084 and the call gets rejected by the intuity. Hang-up.
Call again and dial the extension and it goes through.

I've already checked all of the COS and COR on the voice port stations and the individual stations. We never max out on simultaneous ports for the voicemail.

OK I'm stumped. Can you re-create the problem on all voice ports? If it is just one then maybe it's a single voiceport issue?
