ICS 6.1 with Call Pilot R2 stops answering

Second Call Pilot installed and the ICS System was upgraded from 4.1 to a 6.1. The problem also happened with the original call pilot.
What happens is after a number of days in use calls go to the first port of the system and the system will not answer. Even if the first port is disabled on the ICS the calls still try to go to the first port. The ICS system shows the calls are trying to forward to another port and this doesn't happen (it just goes in a loop). If a second call comes in simultaneously the second call will be answered. If you try to log into voicemail this also will fail. But if a second call in comes in (when you try to log in) it will work. A reboot of the Callpilot will make the system work properly again for a few days then it happens again. Any suggestions?
 
Is wired out correct?
Inside pins for 1 port and outside pins for 2nd port.

Did you replace both CP and software or just CP?

Did you try moving the CP to other ports on KSU?

Are all ports programmed same? without Appear and Ring?

Yes the Pinout is correct
Both the Call Pilot and software were replaced.
The Call Pilot was moved to different ports on the system.
All call Pilot extensions are programmed the same. Although I didn't think that should make a difference. It has been my experience that only service settings (night) and Call pilot ring settings would control the voicemail auto attendant from answering a call.
