How would I include an image file?

I need to email a form (I am using the mail() function) and also include a image (jpeg or bitmap selected from the client's computer) which would be included as an attachment with the form mail. I imagine that this has something to do with the mimes, but I am not sure how to proceed.

Any ideas?

Yes.
Don't try to reinvent the wheel and use the PHPMailer class. It is a great class that has saved me many times from a headache:
