How to Record Review and Approval of Documents

I would like to use Access to document the review and approval of information in forms (Specific records) and possibly documents in ole fields.

I would want the user to review the information and then hit a button to acknowledge they have reviewed the information and either (approve or reject) the item. If they reject the item they should be prompted to enter a reason for the rejection.

What is the best way to do this.

I have the username/passwords in the database already.
 
Assuming that more than one person can review a document or record and that one person can review a dcoument or record more than one time, you'll want to create a table that records the user, the ID of the record or document, a rejection/approval flag, a memo field for recording comments, and a date field to record when the review occurred. If the items being reviewed are stored in more than one table, you'll have to record not just the ID of the item being revieewed, but also something that tells you where to find that record--the table name might be easiest, because then you could actually use that in your sql, if you got fancy.
