How to keep header and footer from repeating in csv export? 1

I have a report that I need to export to CSV. When I do so, however, the header and footer reprints with each row of data.
How can I structure the report so that the header only prints at the beginning, and the footer only at the end? remember, this is an export to csv format. Thanks again.
