Johnpoole's recommendation is on the money in my opinion. Pay the extra money up front to have your provider extend the demarc and put the smart jack wherever it is that you want it. That way, if anything up to, and including, the smart jack fails then the provider is responsible for repairs. You're just responsible for anything on your side of it.
In most cases where a customer wanted an extended demarc what I've seen most often is for the provider to drop a CAT 5E cable from the required customer location down to the building's demarc room. Bypassing the main building riser altogether.
