How do I create a web form that will send an e-mail

Hi,

I need to create a web form for our Intranet that will send about 15 fields of information to an e-mail address. I have been playing about with CDONTS but it seems to be limited to only sending 'To, From, Subject, Body' information.

The form will allow drivers in our company to request vacation time. I have drop-down menus for selecting dates but the other fields are just text boxes.

Ideally, the output would be formatted or, at the very least, come out in the order it is inputted in. I have used the simple ASP code generator from but it won't output information in the correct order.

I have been struggling with this for ages. Is there a web site with an example or can anyone help me.

I forgot to include some technical information:

We are using IIS4 on NT4 Server with the Option Pack installed
We are using Exchange 5.5 as our mail server
