How do I create a submittable web form

We have had IIS4 installed on our NT4 Server for some time now but would like to make better use of it.

My aim is to create a web form which would submit to an e-mail address. The IIS server is for internal use only on our intranet.

How should I go about setting up the IIS server to send e-mails from this form. Is there a form template I can use to develop from?

Once i've got FormMail from Matt's Script Archive, what should i do next?

I've installed ActivePerl from to run the perl but I think I need some software to send the form, i.e. a sendmail equivalent.

Where can I get this software from?

Please help...
