Get "Referenced Memory Error" when using Livelink Explorer

We use Livelink Explorer 4.6.1 with Livelink 9.5. Several of our users frequently get the "Referenced Memory Error" doing various activities within Livelink Explorer. We uninstalled Livelink Explorer from select PCs and people just used Livelink from the browser without any issues. Has anyone else experienced this type of problem with Livelink Explorer installed. We did not have this under LLE 4.4.
 
We do not have the opportunity to upgrade to 4.8 at this time. I did install 4.8 on the client only, and limited testing with Microsoft's ADPlus enabled still shows it throws type 1 access violations -- although I did not create a Referenced Memory Error nor duplicate the explorer.exe crashes as seen under 4.6. As a 4.8 upgrade is not an option right now, I didn't spend a ton of time on it.
 
We have captured diagnostic logs which OpenText reviewed but could not glean anything usefull. We have installed AdPlus from Microsoft and are trying that approach right now.

For the most part, the events appear basically random. The only documented trigger is that some people get it when converting a Word document to PDF.

It would seem there are really two symptons. The first is the "Referenced Memory Error" that some people get, other people report that Livelink Explorer will "hang" "freeze" or "lock-up" and their only course of action is to go to Task Manager and kill it. For those who have the hang or the freeze, they are also primarily imacted by converting a Word doc to PDF using Adobe Distiller. It doesn't happen 100% of the time to those who report the issue and it never happens to some people. My guess is that both camps (Referenced Memory Errors and the "freezes") will have the same solution.
