Form to choose database at runtime startup

What I need is for a user to be able to choose the MDB or MDE file to run on their network station after they deploy the program on thier computer.

I have seen other runtime access programs have a form that lets them browse to the file they wish to run at startup.

Do you know where I can get an example of this form, or where to buy it?
