Form created in dreamweaver doesn't work.

This is my first form so I'm probably missing somthing simple but up to this point it still isn't working. Any thoughts, suggestions would be appreciated.

Make sure the FormMail.cgi has permission to execute on your server.

Wow JT that almost looked like you knew what you were doing!
