Error reading header block

I have an image on tape. The image shows up in the restore window. An images on tape report for the tape shows the correct image name. BUT when viewing the list of media in the library the tape itself shows up as unassigned! The restore fails with an error about reading the header block of the tape. Does anyone know of a way to "fix" the tape or should I just expire the image from the database?
