Enhanced EC 500 not working properly.

I have 2 locations where EC 500 stopped working altogether. I downloaded Avaya's EC install and programming guide, and went through it, looking for any setting that might have changed. Everything in the manual is set as it should be in the Definity. I use 2420 phones. When I press the EC500 button, the screen states "EC500 enabled", but when you call it doesn't forward to the cell. Does anyone have any ideas?
 
Did you set up the off-pbx mappin g to add the station?
 
Some troubleshooting ideas:
-Do you still have a license for it in disp sys cust?
-Do you have available trunking (not busy lines)?
-Cell phone working okay?
-ARS set correctly in off PBX Station mapping?
-10 Digit phone number programmed in the station mapping?
-Check settings in "change off-pbx-telephone configuration-set 1"

-Do you have available trunking (not busy lines)? - Yes
-Cell phone working okay? - My phone is working fine.
-ARS set correctly in off PBX Station mapping? - Trunk selection is set to ARS
-10 Digit phone number programmed in the station mapping? Yes
-Check settings in "change off-pbx-telephone configuration-set 1" - Checked all settings.

Thats the funny thing about this one. It was already working. It just "stopped".

Other things I checked:
EC500 = Y
Extended Coverage / Fwd Admin: Y
ISDN PRI = Y
ISDN-BRI Trunks = Y
COR/FRL = 6
COS: a) Trk-to-Trk override=n; b) Restrict call FWD off net=n

CCRON - Off net coverage enabled = Y
Activeate answer detection on final CRON coverage point=Y
Trunk-to-Trunk Transfers = Y

 
Can you try busy/release on that station that's not working? Then re-test?
 
Well, I've got some further information. When I do a trace on the call, I get the following error: "Denial Event 1643: Off-hook dialing time-out." I spoke with an Avaya tech, and she seemed to think that it was related to programming, but wasn't sure. She was going to check with some engineers and find out what that error meant. I haven't done the research yet myself, but I thought I'd throw it out there to see if anyone else knows.
