Duplicate extension

I have a new IPO with two combo cards in it. When I configured it I noticed in the extension tab that extension 101 was in there twice. I have never had this happen before. All other extension are fine. I have replaced the combo cards and I have defaulted the box and rebuilt the configuration from scratch and it is still showing two extension 101's.

Because of this I cannot renumber my extensions to put my analog at the end and it also will not put the user name on the phone associated with extension 101. That said everything else about the phone is fine like, profile, voicemail, etc...

Any ideas?

9.1 sucks really.
Try rebuilding it starting at R9 then upgrade it to 9.1 and hope it doesn't corrupt the config again (that's what's happening)
Make sure you make a call while it's at 9.1 before starting this though, so it gets locked to that release not R9
