Dialing Out - Call Drop

I have a user with a 6408D+ who sporadically has dialing issues. She has 3 call appearances. Upon attempting to dial out she will begin to dial externally and she will either get a fast busy or the call is dropped and it acts as tho she just picked up the receiver giving her a dial tone again. She had a headset connected, but upon removal the problem continues. Once again this is sporadic.

Has anyone had this happen? Resolution?

Unfortunately I'm not a superuser, I just know enough to be dangerous.

Sometimes, the jack for the headset can cause problems if the info card that slides out right next to it is not all the way seated. The info card is pretty much useless if you want to use a headset, which is a shame, cause it can be a nice place to keep telephone info. If that is not it, I would just trade out the phone and if the phone stays with the problem, replace it.
