dialed number doesn't always ring the number dialed

Customer has a 9.0.1 with a T1. Intermittently the last for digits being dialed aren't being sent. They get a wrong number when on the display it shows the number that they were dialing. In each case I have found the only difference is the last four digits. For example:

Customer dials xxx-xxx-6730 but the call rang to the number xxx-xxx-6773

or another one is xxx-xxx-8049 but the call rang the wrong number of xxx-xxx-8804

This happens on long distance and local calls.

The service provider shows what they are getting is the second number being dialed(wrong number) and not the dialed number by the customer. Any ideas would be appreciated. Thank you.
 
Are they dialing from the speakerphone, headset, or handset? Could an echo of the digits be picked up by the microphone and get added to the dialstring? Have you run a Monitor trace while the call is being dialed?
 
They are using both a headset and handset. I have monitor running now for a trace. I had had it running before but their IT guy was closing it( I have that rectified now).
 
What funkiness do you have in your short codes or ars tables?

You haven't lived until you've been affected by a lack of gratuitous arp.
 
has this always happened from day 1? is this happening on all phones as well?
 
If the numbers posted are real and not only examples I would think TTT is right. There are always repeated numbers...
 
Nothing crazy in the short codes or ARS. Basic setup there.

The problem started about two months ago. Affecting different users and no certain times.

The numbers posted are real.
