Can anyone help shed some light on this please? I deleted a user from an OU in AD. The user was the last one listed in alphabetical order. I highlighted the user, right mouse clicked and selected delete. After confirming my action the whole OU disappeared. I can restore from the System State and link the old e-mail accounts but I'm trying to understand what happened.

apart from a bug i'm not aware of, a case of being sure what is selected when deleting, then finding out that it wasn't exactly that....
It can happen to all of us, just have to look at some questions in the MCPs to know that hardened admins can and do do this sort of thing....
do an authoritative restore b4 its too late..
