A good way I found to deal with issues pertaining to situations where certain emails don't come in, delays in receiving email etc..
(a lot of those really tought to isolate issues that are intermittent)
A good way to deal with it is by a content filter - I use MCafee Webshield which acts as a mail server and receives all inbound email first. Then it forwards it to the exchange box. I can basically tell the moment an email enters the network and track it's IP and other information.
If exchange rejects it, I can tell from the webshield logs. AS a result, we can identify problems that are related to our exchange server vs problems on the senders side.
Rather then screwing around with reverse DNS look up's and security issues that might be hindering your mail services, this is a quick and easy solution to solving those issues.
BTW, webshield also stops spam relay, viruses, bad content and other malicious things from entering the mail server to being with.
