We just experienced an outage with our 2003 exchange clustered environment.
Here is what I pulled from Event Viewer.

Database error 0xfffffd9a occurred in function JTAB_BASE::EcUpdate while accessing the database "First Storage Group\Mailbox Store 3 (exchange cluster)".

This event affected all 3 of our Mailbox Stores and we were forced us to fail over our cluster. We lost email functionality for about 7 minutes while we tried to perform research and decide on a solid plan to bring email back up.

Currently email is back up and everything seems to be fine but we would like to know what possibly caused this outage and how to prevent it in the future.

On an Exchange cluster, if you reduce the checkpoint depth to reduce your failover time, you risk getting this error.

Nothing to my knowledge was change on the Exchange cluster.
