Any ideas about creating custom icons?
I have heard they have to be in Windows icon file format. What exactly does that mean?

It's been a while since I did anything with icons but I seem to remember that they are 16 x 16 or 32 x 32 .bmp files with a .ico extension.

There are a bunch of programs floating around to do it, try googling for Icon editor or similar.

You could try, iconforge, its free and easy to use.
