Creating a Search Form

I am trying to create a search page for my access database application. I am looking for some ideas on how to approach this. I have 5 items that I want to give the option of searching by (ID #, specific Date or date range, Location, Item, Cause). I am having a hard time with the design of the page and how to display the results. I am looking for ideas on what other people may have done or would suggest. All suggestions are welcome.

I recently set up a simple search page. First of all, I set up separate queries for each search field, with [Enter *FieldName*] as the criteria (using *FieldName* for an example only). Then I placed a frame with option buttons onto a form, and a command button to initiate the search. The user selects the field they want to search on by selecting the corresponding option button and then clicks the command button. The command button has code for its click event that checks which option button was selected in the frame, and then runs the query corresponding to the option button. My output was simple, I only wanted a simple query (datasheet) view, but if you wanted you could set up a form and display the results with formatting onto that.
