Continuous Forms involving Checkboxes ??

I have a form that contains various information about a client (i.e. name, phone number, etc.) I have a check box on the form also to add that person's name to a temporary table. But when I click the checkbox for one record, all of the checkboxes for every record are selected. I have tried the "Link Child" and "Link Master" field things out, but I cannot use it because this is a single form, not a subform. Any ideas how I can make it so that each checkbox is separate from the next?

If you bind it to a field, it will not be the same for all records.
