I have inherited a website with a contact us form that some days works and some days doesn't. Usually if I go in and change the name of the form it will start working again. anybody know why ?

Does it use Frontpage Server Extensions to handle the form? Are you the only one that works on the site? Do you sometimes use FTP to connect to the site? Have your repaired FPSE?

How do I know if it uses frontpage server extensions ? I am the only one who works on the site and I do not use ftp to connect.

You would need to ask the hosting company. They just might need to reset. do you have access to the webhosting control panel? Does it have a FPSE administration section?

I do have access to the control panel and it does have FPSE admin section (I think) I'll check tomorrow. I was looking around in control panel but didn't know where to look.

Sounds like you might need to email the support department to ask them or search their FAQs
