Conference call phone not able to do conference calls.

Hello all, I have an issue regarding an Avaya B169 conference phone. When we try and make conference calls it gives us a "wave off" sound that makes the "dee-doo-dee d00" tone. I called Avaya and they said it was a permissions issue but I am not sure where to look for this. The Avaya phone system is a 500 V2 system, the phone manager software is 9.0.6. Any help is greatly appreciated, thank you
 
Thank you for your quick response, yes we are following the instructions properly, verified this with the Avaya rep, wish it were that simple.
 
To set up a conference do the following (max 62 invites):
Make a call, when connected press the "Dial" button and enter the second number to add to the conference, when connected press the "dial" button and dial *47 to add all parties on hold to the conference.
To add subsequent parties press the "dial" button, choose the number to add and when connected press the "dial"button and *47.

The conference button on the phone is for a 3 party conference on the phone itself, pretty useless.
And that will only work if "Call waiting" is enabled and you have multiple appearance buttons setup.
 
I don't know if this applies to the B169, but with the B159 you have to go to Menu > Conf Guide > Settings. Press Ok on Enquiry, change Conference to F*47, Press OK on Return.

I guess the reason for this is the phone is setup for Aura by default.
 
I haven't found that option in the B169 but that can be my error though.
Usually I install at least a 1408/1608 phone next to a conference phone for initiating and administrating conference calls, it is so much easier and you can add/remove parties without disrupting the running conference on the conference phone, a task for the chairman(the man with the hammer) to manage a good conference.
All customers are happy we install it like that.
 
OK just followed John563's suggesterion, was able to find the settings in the conference guide. Changed this to F*47 but was not able to make a successful conference call, still getting the wave off sound. Keep the suggestions coming, something is bound to work. Thanks for all of the help!
 
It must be a user setting, did you:
- enable call waiting?
- program multiple appearance buttons (although these phones have no appearance buttons I noticed on some releases that without multiple button you cannot invoke conferences)
- made a trace with sysmon in order to track down the problem? If not, why not, it is the first thing to do.
