Can't get to the database due to StarUp form 1

I foolishly unchecked all the options on the StarUp form. Now I can't get to the database or the code. Any ideas how I can reset that? Thank you.

Try to open the database while holding the shift-key depressed.
You may also try to play with the F11 key.
