We have 3 digit extensions. When DID calls come in they are routed to the last 3 numbers of the DID and the call rings ok at the phone. When the call goes to Voice Mail the VM says 4838551 does not subscribe to this service. Why doesn't the VM understand the phone's ext of 551?

Your VM is set to look at 4838xxx. Either tweak your VM to look at xxx only or you can build the mailbox alias of 4838551 in mailbox 551.

You're probably getting 7 digits from your carrier on the DID and using a IDC table to strip the first 4 digits for termination on just the last 3, or more likely your LDN is set to 3 digits so the PBX is only looking at those last 3 regardless of the length of the number sent.

We've run into this before during upgrades... Our solution in the past and again today was to put the full 7 digits into every mailbox. Previously we thought it was due to something in the upgrade. On this occasion it happened after updating the patches. (Rls 3.0) There has to be a trick somewhere but we haven't found it yet. The VM is Call Pilot. (Crash Pilot)(Call Pileup) your choice...

There was a "problem" in 25.40 / 25.40B software that sounds like this. There was a patch released that actually fixed it.

Cannot remember the number on it, but if you search on "mail" I think you should find it.
