Call forward whilst set to unconditional with internal/huntgroup calls selected seems on occasion still to ring at the phone forwarding.
Its 4.1 in an SCN environment

ACA - IP Office Implement
ACA - IP Telephony
CCENT - Cisco ICND1
CCNA - Working towards.

The user that you have forwarding set on, is the phone for the extension plugged in? I had the same issue, but it worked after I plugged a phone into that extension.

Oh yeah phones plugged in.
I have seen a post like this previously where scn is concerned but cant seem to find it.

Did you check the VoIP line shortcodes to see if the forward number is not compatable to the SCN codes. In the US, we have people forwarding without the 9 in front or other issues related to the way that the codes were set in the remote side. Couple questions:
Is the issue on the remote system?
Do you know where they are forwarding to?
Does internal forward work?
Is the issue intermittant with the same phone forwarding to the same number?

Could it be that All channels are in use at the time of the call? I ask this because just today I was working on forwarding to cell for a customer. I put the cell number in and forwarding was on but the call still rang at the phone and did not forward. When I checked the number I had forgotten to put in 9 ( my shortcode for ARS).

I am thinking that it might be trying to forward but sisnce it can't it just rings the phone.

I did get a call from a customer also on 4.1 with SCN:
If call an extension (from external) on one site that is set unconditional forward to an extension on another SCN Site that is set unconditional forward alswell but off-switch (Cellphone into the PSTN-network) it will ring the 2nd extension and not going to the CellPhone.
If you dial the first extension from internally all goes fine.

I haven't looked into this but could this be your cause as well?
Because it needs 2 diverted extensions and one even off-switch it doesn't happen that often.
Next week I'll be able to get into this and test it with my 2 4.1 lab-systems.
