This is taken from the Lotus Developer Domain website:
"This special file contains design elements of server-based databases and cross replica journaling information." It is similar to the cache file that a web browser uses to increase performance.

Affirmative, you can delete cache.dsk as often as you wish (when Notes is not active). You might experience a (tiny) increase in load times for the first few documents you open after, but it will soon disappear.
In practice, I find this functionality rather useless and would prefer to have an option for forgetting it. Limiting it to 5Mb is quite enough.
