Anyone use Centerpoint Form Manager

I've got a survey all programmed, but now need to record the voice files. I have an ext. programmed to make form manager recordings, and it works correctly when I call it. It prompts for an extension and password to log in. I have no idea what to put in or where to find the current ones. Is there a default extension/password? Does anyone know how to do this?

I'm looking at page 6-2 of the Conversant System Form Manager User's Guide (585-313-718 Comcode 108474743).
