All lights flashing

I have a IPO 403 that has failed. When I boot it up, all of the LEDs on the front panel flash. Anyone know what this means?

I am unable to get connected to it through the network or maintenance port.

Any idea?
 
disconnect all the phones, ethernet cables and PRI's,

if not that then try removing any internal boards like the PRI or VCM's,

long shot but try a new P/S, and if not that then you may be sunk
 
The only other power supply I have with me is one for an IPO SOE. It has the same voltage and current rating as the one on the 403 so I tried it. When I plug it in, all of the lights come on and stay on (no flash). When I revert back to the original power supply, I get the flash again.

I may try getting another IPO 403 supply before I replace the unit. Subsequent to my orginal message, I noticed that the indicator light on it was not on.
 
this sounds to me like it may have one of the intermediate firmware builds loaded (eg V1.99 used when upgrading form V1.x to V2.x)

I've also had this happen if one of the analog ports got hit with a small surge. In my case, it blew the little chip used for that port, and the blown chip caused a short, thus repeated rebooting. Once I removed the chip, it booted up fine. Dead giveaway is when you touch the chip, it's as hot as the sun (impressive for such a small chip).

It was a pain tracking down the replacement chip, but we were able to find it and replace it with success.
