create a form.. and use wizards to link them..

In Access 2000:
Open front end.
File -> Get External Data -> Link Tables.
Choose back end.
Choose new table.
Click OK.
Open front end.
File -> Get External Data -> Link Tables.
Choose back end.
Choose new table.
Click OK.
