Is there a way to make it prompt a user to type in a reason why he/she is logging onto a server?

Example

Joe Cool logs onto Server1
Joe Cool has to input that he is checking disk space from the console.
Joe Cool is then logged into a text document with date and time he logged on plus a reason.
 
Yes, you can write a script that will prompt the user for the purpose for being on the computer and log that information somewhere on the computer ( a text file maybe) if they do not write to this prompt to you can log them out.

And to implement it, just add it as a logging script, everytime a user logges to the computer they will get that prompt.
