Access Company Data From External Website/Extranet

Hello everyone,
I currently work in the IT department of a medical clinic and in recent days I have been thinking about a project that I would like to take on, and I am trying to get as much information that I can.

I would like to create a website that will allow our patients to access some of their account information online(similar to how banks allow you to login and view transactions, statements, etc). I would like our patients to view information about their account balances and upcoming appointments, and other important stuff. This will be my first time attemping something like this so I would like to get as much information as i can about how to access internal data from an external website, security, and any other information that will be helpful in pointing me in the right direction. I am looking forward to learning a lot from this project, so if there are any programs/software out their that does this, i would prefer not to take that route just yet. Obviously, security is a major concern since i will be dealing with patient information.

The server containing our patient information is a SQL 2005 server running on Windows 2003. We have a PIX Firewall and a VPN Concentrator.

For the patient logins, i was thinking about creating my own database that would keep track of each patients login information and look to that database to verify their login attempts instead of using active directory. I'm not sure if this is a good method or not, but that's what I was thinking about doing. Please let me know if there is any additional information that I need to provide.
