Had this same problem with a BCM 200. I got with Nortel on it and it is a known issue with the 200 and the 400. It ended up being a hardware issue. Had to replace the MSC card and the base function tray. The base function tray ended up having blown capacitors. There is also a patch related to this problem that might help.
Only options I can give you is if you have access to Nortel is to get a hold of them and try getting replacement parts.
