6408D+ Relapse

We HAVE had a relapse.[thought we had it fixed at the end of post 6408D+] After making a few successful station changes (Removing & Adding a 6406D+ to a 24p 2wire DIG card) an attempt at removing a 7406+ station and its associated analog phone and then adding it back FAILED. The 7406+ phone was working on another port before we physically removed it and took it to the closet. We punched it down on the desired port and tried to add it in software. the system reported that the operation completed successfully. Phone appears to be powered up, but no line lights or dial tones fire up. Did a cold start on the system (we have R9i software and the only "reset" options are for "login") all start up diagnostics reported PASS on the restart. Phone is now dark; appears not powered up. We do a <test board 01A##> all tests are ok untill we get to our problem phone, Its test #17 aborts 02. Phone now appears powered but disfuctional. We repunch the phone on a different 4wire DIG card and try again. Same result as above. What else could this be? ANY help would help ....
 
Sounds like a wiring issue to me. Make sure you are wiring 568B.
 
Thanks for the thought busster. But, we have been wiring USOC, 568B & 568A for 20 some odd years, are testing our phones at the cross connect in the closet and have the wiring schema for all the circuit packs, so pretty much know what goes where. :-) Wish it were that simple.
 
Have you tried working this from the port straight off the switch? That will make it easier to find the trouble.

Yes Mike, that was our first move when we encountered trouble. We took a working phone from a different station and took it to the cross connect closet and punched a jack down directly on the port. We tried one port then another on the same card then punched it down on a known working port to verify nothing wrong with the jack we were using or our punch down methodology. We then tried it on another similar card/port. all with the same failed results. We tried this with an 6408d+ on a 2-wire DIG card too. the problem seems to be system wide rather than tied to one card, port, phone or connection. it seems that if you do ANY change to a port cofig, let alone try to assign a differnt DN to it, that it goes stupid and claims that the phone is 02 on test #17 even tho it worked just fine b4 we did the change.
 
Are you using TTI? If so I would disable it and see what happens. One other thing that you may want to try is to put an amphinal cable directly to the backplane of the carrier and wire a jack off the cable itself. Fan out the 25 pair and wire a jack to the different pairs.

This would rule out any cabling problems and point the problem back to the switch. If it still does the same thing then this is looking like a software corruption issue. Their may need to be a reset system? to clear this one up.
