5621 lockup

We have a U.S. customer that has an IPO 3.2 v54 and answers incoming hunt group calls(user is only member of a Group Ring w/ call waiting enabled) with a 5621 w/ EU 24 and Plantronics wireless headset. On 2 occasions,the a= or b= apperarance has "locked up" (the ringing Icon appears and does not allow other incoming calls). We tried rebooting/logging off, but the ONLY thing that has worked is removing the Appearance buttons, merging, then putting them back.

Thoughts?
