TESTS: If the LED on the Set shows the LINE on HOLD for no reason, the wiring is going open between the Set and the Control Unit. If the LED's are going out, then it points to the carrier. THINGS TO LOOK FOR: Set up your laptop to watch SMDR data. Have the client record the TIME of the FAILURE and the EXTENSION that had it. Check the SMDR Log to see if there's a STAR (*) next to the matching record. The STAR (*) means the far end disconnected first.
