    Under System -> Telephony make sure "Inhibit Off-Switch Forward/Transfer" is off. If it still fails I'd suggest looking at what's happening on the system with monitor as it'll provide some insights as to why the call is being disconnected and what is initiating the disconnect.

    What interface card between the 2921 and CS1000? I personally use a VWIC3-4MFT-T1/E1 card to link to my Avaya G650 (which would have the same setup on the Cisco side). If you don't have it already you'll need a PVDM3 module to do the transcoding between IP and TDM.
