Figured out "on hook default path" isn't as important as THIS menu setting: "Telephone Options > Preferences > Headsets > Active Headset Device: - change it from "Wired" to "USB" Also worth noting... on the 1165E phone, you will only see the option for 'Wired' - you will NOT see the...- kf7tsu
