    Thanks, everyone, for your help. Here's what ended up working: Checked CRC box. Un-checked CSU box. All other settings the same as above. D-channel came right up after these changes.

    I haven't been able to try this in person, but the customer says the phone system "lights up like a Christmas tree" when he plugs in the T1 cable with the line set as KWING suggests. However, he installed a dual T1/PRI card in a 406V2 which already contained a dual T1/PRI card. Any thoughts?
