Office97 Toolbar Crashes

(OP)
NT40 client. MSOffice97SP2; OL98

Dr. Watson reports an application error and generates a log. Interestingly, when running Dr. Watson he reports 'No Faults Detected' even though Dr. Watson reported:

MSOFFICE.exe
Exception; access violation (0xc000000005), Address: 0x0141ba8e.

I've tried reinstalling Office, copying MSOFFICE.exe from another PC.

The error occurred after creating an Active Channel on my desktop, then adding the channel to my toolbar.
