extension ringing but user can hear ring

We are using Avaya CS 1000 system one of our digital extension ringing but attender cannot hear the ring sound when call to that extension but in system its shows idle however there is dial tone and can call from that extension out side and if that extension foreword to another set then its ringing if call that extension
I had changed the phone set, Changed TN
when changed TN,extenssion works few weeks than again problem appeared i have had tried all possibilities
Extension's TN is attached
what will be the actual issue? kindly advice

Your printout shows 3 SCR extensions. Do all three extensions fail to ring at the M3904 phone?.

Have you done a IDU on that TN number?. What terminal set is plugged in?.

I've known problems to happen with the wrong terminal type plugged in against what's been configured.
