Trunks on SV8100 not disconnecting

I have an SV8100 with Analog Trunks. They trunks randomly seem to get stuck. We have Function keys on each phone assigned to the trunks and one or two trunk lights will be on even though there are no active calls. We have had Verizon out and they reported no issues on the lines. We also swapped out the CO card on the system. We called NEC support but their only suggestion was that the calls that went to auto-attendant or voicemail were getting stuck but all of their suggestions didn't pan out. If anyone has any suggestions on what this could be or any tips on how to debug this it would be greatly appreciated.

1. Verify with Verizon that a disconnect signal is provided. Analog lines can come with or without disconnect.
2. Make sure that you auto attn. does not have a time out option set to go to leave a message. This lends itself to robo calls getting in and filling up the mail box with silence, but keeping the trunks tied up.
3. Verify that none of your voice mail boxes have been hacked. Look for an out dial option to an international number. Hackers will tie up you lines forever if they gain access. Make sure that any unused mail boxes are disconnected or turned off, and everyone has a password in their mail box.

We did verify with Verizon that disconnect signals are provided. This is not happening on a consistent basis. Most of the time it disconnects just fine.

Are you referring to the timeout option on the dial action table? We do have that set to dump the call into a voicemail box. The max length voicemail is 120 seconds. Shouldn't the line disconnect after that?

Pretty sure everyone has a password. I have seen these things get hacked before and we put a lot of default standard in place to prevent it.

Temporarily change the time out to disconnect. Add an option to the caller to press a digit to leave a message. See if it still has the problem. It's not likely anyone still has a rotory SLT where they need time out to leave a message.

The carriers tell us all the time how Disconnect supervision is set on the trunk only to find out it is not set. Put your butt set on the line make a call and hang up. At this point how long does it take for the line to disconnect. If the line does not disconnect it is a problem with the carrier not your system call the carrier back.
