During an internal conversation, if the original dialer hangs up first the receiver on the other end will get an annoying tone until it times out after about 30 seconds or the other person presses drop.
Have played with busy tone detection with no luck, is there any way to fix this?

talk to the carrier make sure supervised disconnect is on

Do I see that correctly?
If so are these analog stations?
If so there is no way that the phones will release as the analog stations are just a bit different. You could try and set them to IVR in the extension programming that will give a disconnect but then the extension will be still off hook and get a dialtone again.

Give a tech a solution and he will be back tomorrow to ask you the next question, teach a tech how to read the manual and he will be able to solve the problems for a life time.

No, I am talking about an internal call, there are no trunks in play during this call. If you have a couple of Avaya phones in arms reach try it yourself,
ring a phone
answer
hang up the phone that made the call
The destination phone will play the busy tone until drop is pressed

So the person who answered the call did so on the speakerphone? They need to hang-up the phone by pressing speaker button on their end. It won't automatically disconnect when the caller hangs up. The speaker is still active as you have discovered.

Yeah i know, this is what I am tying to fix!

You need to explain to these people that it's not a mobile call.
Basic use of the telephone will circumvent this issue- hang the phone up! It's called 'ringing off'

They wouldn't forget to turn their car off when they drive home, they close the door of the refrigerator, they press 'enter' on their keyboard...

call, answer on loudspeaker, hangup phone that initiated call?

Yes, works fine for me here on 14xx, 16xx, 95xx and 96xx. Have you changed audio path settings or other related telephony settings on the user/handset? :)
