I've created a custom PDF output setting (based on the preset "Press") for hi resolution output. The files are created in inDesign CS5 - CMYK and several spot colours too. I can check the output-preview and it looks OK. When I check the output preview of the PDF (or the film/plate seps ahhh!) they have been converted to RGB. How do I keep the separations in CMYK?

look in your preset at the output options

There you can choose conversion options.

Normally you would choose convert to destination (preserve numbers)

Then you have to choose the destination

Choose for destination your cmyk color profile

and don't include the profile

that should do it

I've tried that - I  looks to be the same result no matter what option from I choose from this menu.

...what output preview are we talking about here?

A third party output preview within a workflow?

...sounds to me like your workflow settings are making conversions to the PDF after the fact the PDF is created (correctly from indesign).

...in no way should indesign be preserving or converting to RGB if you are using convert to destination and a correct cmyk profile...

...if rgb is used in the first place and you choose "no color conversion" then you will end up with RGB...

...the indesign transparency blend space will also be a factor if you are using transparent effects on an indesign page, the whole page will be converted to RGB if you have transparency blend space in RGB and have "no color conversion" set in the PDF export and transparency is set to be flattened in the PDF...
