I run on Windows NT 4.0 and if the icon for the MicroSoft Office Shortcut Bar is in the Taskbar (which is not auto-hide) then when I log out, I get a blue screen of death with some text saying the system crashed. If I exit the shortcut bar before logging out, then the blue screen of death is avoided.  If I have never clicked on the shortcut bar so the icon is not in the taskbar, then the blue screen of death is also avoided.

Has anyone else seen this? Any other workarounds?

No, I have never seen it. If I were you, I would find msoffice.exe, delete it, then reinstall the Office shortcut bar from the CDs.
