I have done this before but my database became corrupt so i had to start over

I made radio buttons with these fields corresponding to each(->):
Active           ->     Space Number    
Not Active
Waiting List     ->     Date Added
I wanted to make it so that if Waiting List or Not Active were clicked then Space Number would have something such as N/A and Not Active respectively in it but if Active were clicked then I would enter a 2 digit number

and if Active or Not Active were clicked then i wanted N/A or Not Active in the Date Added field if it is waiting list then i wanted it to be formated for a date

i have done this a long time ago with 2 forumlas
i thought i used something such as for the 2 calculate fields:
1. If("Status Radio"='Active', "Date Added"='N/A', If("Status Radio"='Not Active', "Date Added"='Not Active',''))
for the Active radio button and:
2. If("Status Radio"='Waiting List', "Space Number"='N/A', If("Status Radio"='Not Active', "Date Added"='Not Active',''))

i thought that's what i put but it doesn't work...
any different approaches or formula's would be appreciated
