Is it possible to import a pdf file into a form report in Reportsmith?  The Reportsmith phone help guys have no idea, & when I import it the same way I would a word document (which has worked fine for me before)  only the adobe acrobat symbol appears in my form report, even if I resize it only zooms. Please help!!

No...but there's alot of third party products (free trials) that will convert .pdf to .doc.  DocSmartz 1.5 does this very, very well - other products out there don't do so well.

Thank you!! I will give it a try.... do you know if there is any way to convert/import a pdf into an excel format?

I just did a search and there's a lot of converter software out there (pdf to xls).  You may need to try several and choose the best since many of these converters don't traslate very well.
