XML to MySQL * HELP!!

I am trying to pull xml data from a .xml file into MySQL. I read about the extractvalue function. Does anyone know how to use this or a better why to extract data. The result will be a findings matrix with approx 6 or 7 columns populated with data.

Please help. Thanks in advance.
